How do I make an appointment?
If you are having an emergency, please contact 911 or go to your local emergency room for treatment.

If this is not an emergency, simply call our office or email us. We will get back with you soon. Phone number, email and address can be found on our "Contact" page.
How long can I expect to wait for an appointment?
Typically we can get you an appointment very quickly. Depending on your availability, most appointments are scheduled to occur within 10-14 days of your phone call. Evening appointments may take 21 to 40 days.
What information do I need to make an appointment?
To make an appointment, please be prepared to tell the secretary your full name, birthday, address, phone number, and insurance/payment information?
Will you bill my insurance?
Yes, we do bill insurance plans, whether or not we are a provider to your particular insurance plan. If we are a provider to your plan, copays and deductibles are due at the time of service. However, if we are not a provider to your plan, you will be responsible for the full amount of your bill at the time of service.
How is payment handled?
Payment is due at the time of service. If you have an insurance plan, the amount due, copays and deductibles, is determined by your insurance plan. If you have an insurance plan, to which we are not a provider, then the full amount is due at time of service. If you do not have an insurance plan, then the full amount is due at the time of service.
What methods of payment do you accept?
We accept cash, check, MasterCard and Visa.
Are there any policies of which I should be aware?
